Question or page
Exact wording or URL. Include any follow-up question.
Context
Engine or model, language, location and session. Mark unknown settings.
Saved observation
What appeared? Where did you save the answer, screenshot or response?
Source & conditions
Cited URLs, source version, limits on the claim and who checked it.
What changed
The specific edit or event, when it happened and the previous version.
Next check
What to repeat, under which conditions and when.
What remains unknown
Other changes, missing settings or a result you cannot yet attribute.
Keep the observation narrow. A changed answer is not proof of cause; a mention is not a website visit; working pages are not proof of indexing.
